1. How do I add a spouse or dependent to my insurance?
To add a spouse or dependent to your insurance, please contact the Insurance Department. They will be able to assist you and walk you through the necessary steps.

2. When can I make changes to my insurance coverage?
Changes in coverage can be made during the annual open enrollment or if you have a qualifying event.
Examples of a qualifying event are as follows: marriage, birth or adoption of a child, death, divorce, and loss of coverage. If you have a question about a potential qualifying event, please contact the Insurance Department within Human Resources.
Changes due to a qualifying eventmust be submittedto the Insurance Department within30 days.Requested changes beyond the 30-day window will be denied.
3.Where is my health insurance card?
After enrolling, you should receive your Blue Cross Blue Shield of Louisiana member card within 10-14 business days. If you don't receive your member ID card within the normal time frame, please contact BCBSLA customer service at 1-800-363-9150.
Leave of Absence
1. What is FMLA leave?
FMLA (Family and Medical Leave Act) is a federal law that provides unpaid, job-protectedleave for up to 12 weeks, as certified by a healthcare provider, for qualifying events. To be eligible for FMLA leave, employees must have worked with APSB for at least 12 monthsandhas at least 1,250 hours of service during the 12 month period immediately preceding the leave.
2. When can I use FMLA leave?
FMLA may be granted for the following reasons:
- for the birth of a son or daughter, and to bond with the newborn child;
- for the placement with the employee of a child for adoption or foster care, and to bond with that child;
- to care for an immediate family member (spouse, child, or parent - but not a parent "in-law") with a serious health condition;
- to take medical leave when the employee is unable to work because of a serious health condition; or
- for qualifying exigencies arising out of the fact that the employee's spouse, son, daughter, or parent is on covered active duty or call to covered active duty status as a member of the National Guard, Reserves, or Regular Armed Forces.
Up to 26 unpaid work weeks may be granted to care for a covered service member with a serious injury or illness.
3. When do I need to submit a doctor's note or an FMLA packet?
After 6 consecutive absences, a doctor's note is required prior to your return to work. After 9 consecutive absences, an FMLA packet is required.
4. I don't want to use FMLA leave. Can I use my sick time?
The FMLA packet isrequiredafter 9 consecutive absences. While on FMLA leave, APSB uses your sick time balance. Once/if your sick time balance is exhausted, your extended sick time will be used. You will receive 65% of your daily pay while on extended sick time. This will provide you with some compensation while on FMLA leave.
Retirement
1.Do you know how much my retirement check will be?
This will be determine by the retirement system(TRSL, LSERS, etc.). This cannot be determined by APSB. This formula is determined by the retirement system. The following documents will link to the TRSL and LSERS benefit estimator.
2. Can you tell what date I can retire?
Yes, please send an email to retirement@apsb.org and one of our HR retirement clerks will be able to calculate this approximate date for you. You can expect a response within 48 hours of sending this email, due to research that may need to be done by the retirement clerks.
3. What about my insurance with APSB when I retire?
This will depend on the years you have been enrolled in health insurance. Please contact the insurance department for additional information (employee.insurance@apsb.org
4. Do my benefits change when I enter DROP?
No, benefits do not change when you enter drop.
5. What is 403(b)?
403(b) is a voluntary retirement plan in addition to your state retirement with TRSL, LSERS, or LASERS. You can select your pre-tax contributions and adjust them at any time. This planallows employees to invest a portion of their salary for future retirement use.
6. When can I enroll in a 403(b) plan?
Eligible employees may enroll at any time. You do not have to wait until open enrollment or a qualifying event.

1. How do I submit a name or address change?
You can make changes throughthe employee service portal. Please find the instructions to change your address here.
2. What documents do you need for a name change?
Marriage Certificate/ divorce decree, drivers license, social security card
3. How do I make changes to my federal or state taxes?
To update your state taxes please complete the state L4 formand send it to human.resources@apsb.org.
To update your federaltaxes please complete the federal W4form and send it to human.resources@apsb.org.
4. Why are my taxes being withheld from my check?
We input the information listed on the W4 and the L4 forms. We recommend speaking to a tax prepared as they can give you more information as to why your taxes are not being withheld.
